Please fax no later than 28 April. NOTE: The form cannot be filled in on line and no photo is needed. Please keep the original of the form and bring it when you collect your badge. Badges must be collected from the United Nations Security Identification Office at Villa Les Feuillantines, 11-13 Avenue de la Paix, between 8.00-17.00. Villa Les Feuillantines is located about 50 meters north of the Place des Nations on Avenue de la Paix on the left hand side of the street as you approach from the Place des Nations. NOTE: Badges for the conference can be collected from 2nd May (one day in advance) or on 3rd May in the morning.
